Library at Home Service Agreement
By submitting this form:
You confirm that you are an Allen County resident that is unable to regularly use a physical library or curbside service.
You agree to borrow library materials that are selected in coordination with Library at Home staff and delivered once each month to your residence.
You agree to return library materials in a timely fashion.
You understand that fines will not accrue for overdue material but lost or damaged items may result in fees or suspension of service.
You agree to make Library at Home your primary library contact for all library-related requests.
